PARENT/GUARDIAN PERMISSION FORM FOR YOUTH PARTICIPANT

This form must be completed by all youth participating in overnight activities, field trips, events requiring group transportation, and any other events hosted at Isaiahs Hub, where it is deemed necessary by the event coordinator(s). Be sure to complete all sections and sign where requested!


Parental Permission and Release of Liability
I hereby give my son/daughter (named above) permission to participate in the event listed. Although the event coordinator(s) will use the utmost precaution in guarding the health of the above participant and preventing accidents, I release them from any liability in case of illness or injury as a result of this activity.

After School Program
Providing a fun and safe environment for every youth to participate in games, activities, meet new friends, join our weekly discussion hour where we cultivate new ideas and learn Core life skills, Moral and Integrity skills, Key Principles Including Leadership & Ethic.
Free Summer Program
Isaiah’s Hub 2024 Summer Camp offers a hands-on learning experience for children aged 7 to 14, focusing on core life and occupational skills. The camp, running from June 10th to August 18th, includes free breakfast and lunch and aims to foster a welcoming and inclusive environment.
Character Youth Initiative Program
We offer after school activities and snacks, kids can play in the gym, they have computer access, video games, movie room, team game room, basketball open gym, teenager spark zone, free tutoring, home work study rooms are available. Robotics and steam learning available.
Academic Program
Our after school tutoring program features small group or individualized sessions, tailored to each student's unique needs and goals.
After School Tutoring Program
85% of students end up reading at or grade level or higher — showing significant literacy improvement.
B Elite Health and Wellness Program
Our health and wellness program is open to all young people in our community, regardless of age or fitness level.
Hair Stylist Program
As part of our cultural arts program, “Blessed by Miss” focuses on empowering underserved communities by teaching them the art and skill of hair braiding, rooted in ancestral traditions. Our goal is to uplift black and brown communities by equipping them with skills that foster confidence and self-awareness. We believe that when individuals are self-aware and understand their heritage, they can confidently excel in any task.
Career Readiness Program
Through our program, young people have access to a variety of activities and resources, including college application and financial aid assistance, resume and cover letter writing workshops.
Entrepreneurship Program
We offer a range of activities and resources, including T-shirt creation and design, to help young people develop the skills and knowledge they need to succeed as entrepreneurs.
Hispanic Prosperity Program
The Hispanic Prosperity Program’s mission is to decrease the gap between the Hispanic population and their fellow community members.
Theology Program
Where students engage in learning about the word of God if they desire, and learn more about the Bible and what it teaches.
